Job Description
​Position Responsibilities
The Behavioral Health Clinician has the following clinical responsibilities
and duties:
·     Provide
brief or extended Psychosocial Assessments focusing on the patient’s present
situation, including medical, mental/behavioral health, substance abuse,
socio-economic and immigration components, current needs, and relevant
information from patient’s history.
·     Performs
crisis interventions and risk assessments for suicide and danger to others due
to mental illness or grave disability.
·     Utilizes
a participant-centered, strengths-based model of care, provides
insight-oriented, behavior modifying, and/or supportive psychotherapy to
individuals, families, and groups of patients, generally up to 10-12 sessions.
·     Uses
evidenced-based models for care, including but not limited to: CBT,
solution-focused therapy, harm reduction, motivational interviewing, and health
psychology.
·     Utilizes
depression screening tools (PHQ-9) and other screening tools as appropriate
such as GAD-7. Follows the SAMHSA SBIRT (Screening, brief intervention,
referral and treatment) model for patients with potential substance use
disorders.
·     Assists
clients/patients with symptom reduction (e.g. insomnia, anxiety, depression,
and post-trauma symptoms, stress management, emotional dysregulation) through
talk therapy, case management and coordination with PCP or psychiatrist if
necessary.
·     Provides
health and behavior assessment and intervention to client/patients with chronic
disease or other health problems that are having difficulty managing their
disease due to psychological, behavioral, cognitive, or social factors.
·     Creation
of and linkage to already-existing groups that address recurrent referrals
(e.g. psycho-education groups, substance abuse, coping with chronic pain, etc.)
·     Linkage
to intra-agency components that address lifestyle adjustments or concerns, such
as dietary changes, diabetes management, or medication adherence
·    Â
Linkage to appropriate community resources, such as child,
family, and educational resources, immigration resources, employment and
disability resources and maintain knowledge of and relationships with these
resources.
·    Â
Linkage to County Behavioral Health System for patients with
severe mental health needs and substance abuse treatment needs, and provision
of behavioral health services until patients are connected.
·    Â
Ongoing case management to address psychosocial issues which are,
or could potentially, impact a patient’s health or behavioral health.
·    Â
Accurately records patients’ assessment, behavioral health
needs, treatment, and referrals in medical record according to established
protocols.
·    Â
Actively utilizes the Health Registry to ensure patients’
follow-up, track clinical improvements and develop written reports.
·    Â
Works closely with the extended Behavioral Health team
throughout MNHC to ensure standardization and quality. Engages in regular
meetings as organized by the Behavioral Health Director or Manager and
participates in MNHC-wide Behavioral Health initiatives and quality management
projects.

Education, Knowledge, Skills, and Abilities
Â
·    Â
Master’s
in social work, Psychology or Marriage and Family Counseling from an accredited
program or university.
·    Â
Must have
a LCSW, PsyD/PhD or LMFT California license.
·    Â
Bilingual
- Fluent in English/Spanish required.
·    Â
Minimum of
2 years providing social work, behavioral health services to a diverse
population.
·    Â
Experience
working with evidence-based brief counseling models.
·    Â
Experience
in primary care setting preferred.
·    Â
Experience
with Latinx population preferred.
