Join the Community Health Center as an Oral Health provider within a Patient-Centric and Integrated Healthcare Model consisting of Primary Care; Dental, Medical, and
Behavioral Health professionals, serving our underserved patients in need of
access and quality care.
We offer a solid compensation package with a substantial salary,

Salary: $145,000 to
$165,000 DOE, with Sign-On / Relocation funds available
Flexible Schedule: 4x10hrs M-F,
4x9hrs M-TH + ½ Fridays, or 5x8hrs
Paid Time Off: 44 to 52 paid
days off each year!

96 hours/yr (12 days), up to 160 hours/yr (20 days)
*Sick Time –

Summary of
Position: The Dental Provider is responsible for giving
comprehensive quality care to their patient caseload within the scope of
General Dentistry. This position will identify, diagnose, and treat various
oral diseases that present, as well as perform general oral surgical procedures
when applicable. The position will pursue the necessary steps toward
appropriate treatment plans for each patient.
Required
Education and Experience:

Possess or be able to obtain a Doctorate Degree (DDS or DMD) in General
Dentistry.
*
Board Certified in General Dentistry.
*
Possess or be able to obtain and maintain an active Minnesota Dentist License

1 year of experience as a General Dentist

Experience or desire to work in an Integrated Rural Health Center
Supervisory
Responsibilities: This position may indirectly oversee
and direct Licensed Dental Hygienists and Licensed Dental Assistants.
Mission/Focus:
We
maintain a Practice Team Approach to “Patient Centeredness.” The employee works
as a team with providers and staff to support the Patient-Centered Medical Home
approach to patient care.
Essential
Functions:

Comply with HIPAA requirements and standards to guarantee patient
confidentiality.
*
Provide comprehensive dental treatment of patients that present at the
designated clinic location(s).
*
Perform oral exams following routine hygiene cleanings.
*
Identify, diagnose, and treat oral complications utilizing best practices and
treatment plans.
*
Accurately read and document oral X-ray findings.
*
Perform minor surgical procedures such as extractions and fillings.
*
Create dentures and mouthguards for patients.
*
Educate patients on the maintenance of good oral hygiene.
*
Document patient visit notes in the Electronic Dental Records system.
*
Develop positive working relationships with dental staff, dental providers, and
specialists within local communities.
*
Communicate patient care goals and objectives with colleagues and dental staff.
Collaborate on best practices toward goal implementations.
*
Attend required meetings, including but not limited to Quality Improvement and
Provider meetings.
*
Adhere to the health center’s patient care model, mission, and objectives for
quality primary care.
*
Maintain certifications such as but not limited to MN Licensure, DEA
registration, and Board requirements to continue performing the position of
Dental Provider at designated clinical location(s).
*
Perform a minimum of six (6) hours of direct patient contact for each scheduled
clinical shift.
*
Participate in preceptor opportunities with various learning institutions for
dental students.
*
Physical presence at the workplace is an essential function of the job.
*
Perform other typical dentist duties as assigned.
