POSITION
SUMMARY:
Our
FQHC provides team-based care consistent with the principles and standards of a
Patient Centered Medical Home (PCMH).
The Director of Nursing & Care Management plays a major role
in supporting the mission of the health center within our community through
collaboration with government, private sector, community, and health center
partners. The successful Director of Nursing & Care Management develops,
implements, and monitors the delivery of high-quality medical care with an
emphasis on patient experience and access to care.
Combining administrative and clinical time,
the Director of Nursing & Care Management develops and manages
services while promoting health through education, collaboration, and outreach
activities to our patients. The Director of Nursing & Care Management will
be responsible for the registered nurses, nurse care managers, medical assistants, and other support
staff. The Director
of Nursing & Care Management will provide direct patient care to cover vacancies,
and staff time-off as needed, while having an active role in defining staffing
models. Remaining time will be dedicated to operational and administrative
functions that aid the Chief Medical Officer in meeting productivity, process, and outcome goals consistent with our
Health Center’s strategic plans.

Essential
Functions Statement(s)
ü Responsible for directing, organizing, and supervising the work of the Registered Nurses, Medical Assistants, Nurse Care Managers, and
other support staff.
ü Co-create care management processes with the Chief Medical Officer
according to operational and quality outcomes goals including protocols to
address the management of patient total cost of care to maximize reimbursement
and minimize financial risk. Coordinate continuity of patient care with
external healthcare organizations and facilities including the process of
hospital admissions and discharge and referrals from primary care providers to specialty care providers for care management patients.
ü Directs referral processes, prioritizing needed referrals for
patients actively engaged in care management programs and to minimize delays in
access for all patients. Demonstrate leadership through ensuring clear,
convenient, and reliable communication processes between providers and the care
management team including longitudinal care plans and care coordination. Supervise, coach, and mentor staff within functional areas of responsibility and regularly encourage
or arrange educational opportunities.
ü Collaborates with human resources in the interviewing of and
recommendation to hire new staff.
ü Ensures appropriate staffing
levels in a fiscally responsible manner necessary for uninterrupted patient
care during hours of operation.
ü Conducts annual performance reviews for supervised staff, with regular
feedback to staff
according to our FQHC accepted standards. Performs
and/or participates in competency testing of staff to assure the highest quality
of patient care and culturally appropriate service. .
ü Make recommendations for corrective action up to and including
termination in collaborating with human resources and ensure appropriate
documentation and follow-through of any resulting performance improvement plan,
issues, and concerns. Verifies, audits, and authorizes time sheet approval for
payroll processing.
ü Develop and provide in collaboration with the Chief Medical
Officer, department related in-service training activities for medical staff.
ü Partner with IT on NextGen/EMR improvements and enhancements.
ü Collaboratively innovates with our Health Center Quality and Leadership
team to evaluate data and implement performance improvement plans for
identified organizational priorities including quality, outcomes, safety, and
business continuity.
ü Meets departmental quality
and productivity goals in support
of our health center strategic
plan and as determined with the Chief Medical Officer through
review, implementation, and reinforcement of applicable policies and
procedures.
ü Works in collaboration with the Safety Committee in developing and
implementing safety standards and regulations for equipment, supplies,
implements safe practices in accordance with pertinent state, federal, and OSHA
regulations. Works with Chief Operations Officer and Chief Compliance Officer
to ensure that policies and procedures are followed. Possesses and refreshes
knowledge of state, federal, and OSHA regulations, and implements policies and
procedures to ensure compliance with regulations and guidelines.
ü Complies with federal and health center requirements in the areas
of protected health information and patient
privacy.
ü Attend and actively
participate in supervision, training, and other
meetings as assigned.
ü  Maintain strict confidentiality in all matters
ü Operate within the scope of the Health Information Portability and
Accountability Act to safeguard the privacy of protected patient health informationÂ

POSITION
QUALIFICATIONS
ü Graduate of a school of nursing. Bachelor's Degree required; Master’s
Degree preferred.
ü Current licensure with the State of Rhode Island as a Registered Nurse.
ü Experience in chronic
disease management, community nursing, or care management.
ü Managerial experience in a community health setting is preferred.
ü Familiarity with 340b Federal program is a plus
ü CPR certified and knowledge of basic first aid.
ü Cultural sensitivity necessary
to work with a diverse
patient and staff
population.
