A well-established Community Health Center in San Jose is seeking a dedicated Primary Care Physician to join its collaborative care team. This is an outstanding opportunity for a provider who is passionate about delivering high-quality primary care while making a meaningful impact in a diverse and underserved community.

Position Highlights

Work-Life Balance & Practice Environment

  • 36 clinical hours and 4 administrative hours per week
  • Outpatient-only practice
  • No obstetrics
  • No inpatient responsibilities

  • Strong referral and care coordination resources
  • Team-based approach designed to maximize provider efficiency and patient outcomes
  • Advanced clinical technology and workflow support

Responsibilities
  • Provide comprehensive outpatient primary care services to patients across the lifespan
  • Conduct physical examinations, assessments, and diagnostic evaluations
  • Diagnose and treat acute and chronic medical conditions
  • Develop and implement individualized treatment plans
  • Provide preventive care, wellness counseling, and patient education
  • Support chronic disease management through patient education and self-care guidance
  • Coordinate specialty referrals and follow-up care as appropriate
  • Complete patient forms and clinical documentation in a timely manner
  • Participate in quality improvement initiatives, peer reviews, and clinical audits
  • Collaborate with physicians, advanced practice providers, behavioral health professionals, and support staff to deliver high-quality patient care
  • Support organizational goals focused on improving access to quality healthcare services

Qualifications
  • MD or DO degree from an accredited medical school
  • Current California medical license or ability to obtain licensure
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine
  • Active DEA registration or ability to obtain
  • Strong commitment to patient-centered care
  • Excellent communication and interpersonal skills
  • Ability to work effectively within a multidisciplinary team environment
  • Passion for serving diverse and underserved communities
