OVERVIEW: The
Nurse Practitioner / Physician Assistant works under the supervision of the Medical
Director and Lead Physician to provide direct patient care. The Nurse
Practitioner / Physician Assistant works as part of a multidisciplinary team
composed of physicians, medical assistants, mid-level providers, a Clinic
Manager, community health workers, and administration to provide quality
comprehensive care to our patients. Our clinic is a Federally Qualified Health
Center providing comprehensive patient-centered primary care services including
medical, dental, substance abuse treatment, mental health, and outreach services
to low-income, multi-ethnic, uninsured, and underinsured populations.

HOURS/SITE: Full
time, 40 hours per week. Evening
hours may be required. The position is located in W. Sacramento.

RESPONSIBILITIES:

1.           Work
in outpatient family practice community health center with scope of services to
include primary care, reproductive health and pediatric care. Perform all aspects of health care as
directed by physician including health history, physical examination,
diagnosis, treatment, and health education for all age groups from newborns to
geriatric care.
2.           Order
labs as needed and specified by physician.Â
Interpret lab findings for infants, teens and adults.
3.           Prescribe
medications as directed by physician.Â
Current knowledge of pediatric dosages.Â
4.           Complete
charting of all patient medical records in electronic medical records.
5.           Request
appropriate consultation with physician as needed.
6.           Provide
patient case management (referrals and follow-up care) as needed. Communicate follow-up information and high
risk conditions to physician and coordinate care plans with staff.
7.           Participate
in quality improvement activities as needed.

MINIMUM POSITION REQUIREMENTS:
Education: Completion of an accredited Nurse Practitioner.
Current California Nurse Practitioner or Physician Assistant license. Current First Aid/CPR required.Â
Experience: Experience providing primary outpatient care
including pediatrics, reproductive health and family medicine. Experience
working in a clinic setting and a demonstrated commitment to comprehensive care
for low income clients preferred.

Special Skills/Equipment:Â
Bilingual Spanish/English preferred.Â
Ability to operate all equipment used in the clinic sites. Demonstrate skills to provide age-appropriate
care to clinic patients including principles of growth and development over the
life span. Ability to assess data reflective of patients’ age-specific needs
required. Must be able to maintain
current credentialing and privileges for Our FQHC and contracted health plans.
Experience using electronic health records strongly preferred.

written/verbal instructions required. Ability
