Family Medicine Physician – Community Health Center (FQHC), Bay Area, CA

This non-profit Federally Qualified Health Center (FQHC) serves communities across the East Palo Alto, Palo Alto, Mountain View, and Sunnyvale areas. The organization provides integrated primary, preventive, behavioral health, and social services to a diverse and underserved patient population.

Position Summary

The Family Medicine Physician provides comprehensive, patient-centered primary care to patients across the lifespan in a community health setting. This role focuses on delivering high-quality care in a team-based, mission-driven environment that addresses both medical and social determinants of health. Key Responsibilities
  • Provide full-scope outpatient primary care, including preventive, chronic, and acute care
  • Diagnose and treat a wide range of conditions across pediatrics, adults, and geriatrics
  • Collaborate within a team-based care model including medical assistants, behavioral health providers, and care coordinators
  • Participate in care coordination and population health management
  • Maintain accurate and timely electronic health record (EHR) documentation
  • Support and engage in quality improvement initiatives
  • Deliver culturally competent care to a diverse patient population

Qualifications
  • MD or DO degree from an accredited institution
  • Board certified or board eligible in Family Medicine (or Internal Medicine)
  • Active or eligible for California medical license
  • Experience in community health, FQHC, or underserved settings preferred
  • Bilingual in Spanish strongly preferred

Practice Environment
  • Mission-driven FQHC focused on underserved populations
  • Integrated care model including medical, dental, behavioral health, and social services
  • Diverse patient population including Latino, African American, and Pacific Islander communities
  • Emphasis on whole-person, patient-centered care
  • Support staff includes two medical assistants per provider, with approximately 80% Spanish-speaking
  • Productivity expectation of approximately 10 completed appointments per 4-hour session
