Internal Medicine Physician

Full-Time - Expected Schedule: Full-time, 40 hours per week

Description

A respected Federally Qualified Health Center in Eastern Connecticut is seeking a highly skilled and compassionate Internal Medicine Physician to join its collaborative care team. This organization is committed to providing person-centered healthcare that improves the health and well-being of individuals throughout the community.

The physician will provide comprehensive medical care to adult patients, including diagnosing and treating acute illnesses, managing chronic conditions, and supporting preventive health and long-term wellness.

Responsibilities
  • Provide patient-centered medical care in accordance with professional standards, clinical guidelines, and organizational policies.
  • Review medical records, examinations, reports, and test results to diagnose and manage patient conditions.
  • Clearly communicate clinical findings, recommendations, and treatment plans to patients.
  • Ensure continuity of care through participation in an on-call rotation and provide backup coverage for other primary care providers when needed.
  • Make appropriate referrals and follow up on patient education, counseling, and self-care management strategies.
  • Address patients’ comprehensive needs through preventive screenings, treatment of common health concerns, and collaboration with integrated care services, including behavioral health, dental, and case management.
  • Participate in interdisciplinary case conferences and collaborate with other departments and community partners.
  • Identify and respond appropriately to high-risk situations, including notifying the appropriate agencies when necessary.
  • Assist with the development of health risk management protocols.
  • Participate in clinical operations planning and initiatives that address community health needs, epidemiological trends, and barriers to care.

Requirements

Education and Experience
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
  • Strong interest in community health or public health
  • Patient care management experience

Licenses and Certifications
  • Active or eligible Connecticut physician license
  • Valid DEA license
  • Current BLS and ACLS certifications
  • Board-certified or board-eligible in a primary care specialty
  • Ability to maintain credentialing requirements for acute care hospitals, third-party payers, and applicable accrediting organizations
