The Physician is responsible for delivering a wide range of primary care services within their field of expertise. This includes diagnosis, treatment, care coordination, preventive care, and patient health maintenance. As part of a Patient-Centered Medical Home (PCMH) team, the physician should be familiar with the philosophy, goals, and objectives of the PCMH. This is position is 100% outpatient with no weekends and call about 1-2 a year!

Job Duties:
- Perform comprehensive physical examinations and obtain a complete medical history from patients and families.
- Order routine and diagnostic lab, radiological studies, and other necessary tests, and interpret results.
- Collaborate with the healthcare team to establish care priorities, manage care, and coordinate efforts for desired outcomes.
- Coordinate care with consulting physicians, community resources, hospitals, and ER or Urgent Care facilities.
- Participate in pre-visit huddles to ensure smoother workflow.
- Identify barriers to meeting treatment goals and inform the patient care team of their impact on the care plan.
- Foster collaboration with other healthcare team members.
- Ensure care coordination, referral management, and efficient clinical workflow processes through regular meetings with clinical staff.
- Provide clinical care using a team-based approach centered on patient needs and individual preferences.
- Participate in peer reviews, quality assurance activities, provider meetings, and other clinical meetings as required.
- Maintain confidentiality and privacy in accordance with HIPAA guidelines
- Stay current with professional trends and engage in continuous professional development.
- Demonstrate professional conduct at all times.
- Maintain accurate and timely documentation of patient encounters.
- Assess, diagnose, implement treatment, evaluate, and monitor patient health status within the PCMH framework.
Knowledge/Skill Requirements:
- Hold and maintain a current South Carolina Medical License.
- DEA Certification (Preferred)
- Board Eligible or Board Certified as an MD or DO from an accredited medical school.
- Maintain Continuing Medical Education credits as required by the South Carolina Medical Board.
- Possess a current Cardiopulmonary Resuscitation (CPR) certificate (ACLS or Advance Life Support within 12 months of employment, if applicable).
- Demonstrated experience as a qualified primary care physician, including satisfactory completion of residency training in family or general practice of medicine or a rotating internship.
