Job Description


What is PACE?

PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age requiring nursing home level of care which aims to keep participants living at home in the community. PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.




The Medical Director of PACE is responsible for providing clinical leadership for the care and treatment of participants served by Esperanza’s PACE. The Medical Director provides primary care to participants as well as clinical direction and guidance to the PACE interdisciplinary team (IDT) to ensure quality care




PRIMARY DUTIES AND RESPONSIBILITIES




Administrative:
  1. Work cooperatively with the Vice President of PACE to ensure quality care delivery to all participants, affirm the PACE mission, and educate team members on the PACE service delivery model.
  2. Conduct outreach and business development strategies to educate the community and help grow the PACE organization.
  3. Serve as a leader in the presentation of programs at regional, state, and national meetings and represent PACE at appropriate local, state, and national forums.
  4. Oversee PACE Quality Improvement (QI), working collaboratively with the QI Coordinator to identify, monitor and track quality improvement initiatives.
  5. Collaborate with IDT to address PACE Participant appeals, grievances, and service determination requests and lead towards a culture of safety.




Clinical:
  1. Provide primary and preventative care to PACE participants.
  2. Provide clinical expertise and participate in IDT decisions regarding diagnosis and medical management of participant needs, assuring that the highest quality of medical care is provided and that continuity and comprehensiveness of care is maintained.
  3. Establish and implement a preventive care program as appropriate for participants enrolled in the PACE program.
  4. Manage and round on PACE participants admitted to long-term care facilities or acute in participant facilities.
  5. Conduct family or caregiver meetings as as needed.




REQUIREMENTS
  1. MD or DO from an accredited medical school. Board Certified in Internal Medicine or Family Practice. Fellowship training in geriatrics preferred.
  2. License to practice medicine in the State of Illinois.
  3. Two years’ experience as a medical doctor.
  4. Must have at least one year of experience working with a frail/elderly population.
  5. Ability to make home/residence visits.
