‚ÄčJoin our team as a Primary Care Physician!




Position Purpose:




As a Primary Care Physician, you'll provide essential medical care services to pediatric and adult patients, with some cases involving OB/GYN care. Operating primarily in an outpatient setting, you'll focus on preventive health measures and healthy lifestyle promotion. Additionally, you'll contribute to program planning both within our Health Center and the wider community.




Primary Duties and Responsibilities




1. Patient Care Management: Conduct patient histories, perform physical examinations, make accurate clinical diagnoses, prescribe treatments, and provide patient education.




2. Productivity Standards: Maintain a productivity standard of at least 2.7 patient visits per hour of ambulatory time worked.




3. Patient Satisfaction: Promote patient satisfaction by considering environmental, social, and cultural factors influencing health.




4. Quality Improvement: Participate in quality improvement activities, including chart documentation, peer review audits, and designated projects.




5. Evidence-Based Practice: Practice evidence-based, cost-effective primary care within a team-based model.




6. Diagnosis and Referrals: Make appropriate diagnoses, prescribe medications, and refer patients according to medical practice guidelines.




7. Preventative Health: Develop and implement age-specific preventative health schedules for patients.




8. Continuity of Care: Build and maintain a patient practice, ensuring continuity of care through follow-up appointments and referrals.




9. Continuing Medical Education: Maintain clinical competency through regular Continuing Medical Education and provide documentation as required.




10. Hospital Liaison: Maintain liaison with partner hospitals' medical staff, attend meetings, and fulfill teaching duties as required.




11. On-call Duties: Take on-call responsibilities for organization patients as assigned by the Medical Director.




12. Education and Training: Play a role in educating and training medical professionals and accept teaching assignments as needed.




Non-Essential Duties and Responsibilities:

1. Participate in community health education and promotion activities.

2. Attend at least one health fair annually.

3. Contribute to the preparation and implementation of the organization's Strategic Plan.

4. Perform other duties as assigned.




Requirements:

- Education: Graduate of an Accredited Medical School.

- Licensure/Certification: Current Illinois State License, Controlled Substance License, and DEA Registration.
