JOB TITLE: Family Medicine Physician

 

REPORTS TO: Associate Medical Director of Assigned Clinic

 

OVERVIEW: The Family Medicine Physician works with a team of health care staff to provide outpatient primary care services. The physician works as a part of a multidisciplinary team composed of the clinic manager, advanced practice clinician, integrated behavioral health clinicians, medical assistants, case managers, nurses and health educators to provide comprehensive care. Teaching and precepting of medical students and family medicine residents is also encouraged.

 

Our clinic is a federally qualified health center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, mental health and outreach services to individuals and families who are uninsured and underinsured and from historically marginalized groups and populations.

 

SCHEDULE: Full-time or part-time/24-40 hours per week. Evening hours may be required. Position(s) are located in W. Sacramento, Woodland, and Davis.

RESPONSIBILITIES:



  1. The following reflects management's definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time with reasonable accommodation.
  2. Serve as Primary Care Physician to a panel of patients. Patients (aged newborn to 90+ years) may present a wide variety of health problems requiring diagnosis and the selection and management of appropriate care. Such care would include, as necessary, admission to institutional care, referral to specialists, and the coordination of these and other types of care through diverse resources.
  3. Coordinate and provide appropriate care by mean of initial diagnosis and treatment, obtaining second opinions as necessary, consultation with referral specialists and followup of care to assess the results of the primary care, medication regimen and special treatment within the framework of integrated continuous care.
  4. Record completely all patient visits, treatment, communication with patients, treatment, and consultation reports in the electronic medical record.
  5. Participate in the Our FQHC utilization review process.
  6. Participate in the quality assurance program and collaborate on quality improvement activities.
  7. Provide consultation to nurse practitioner and physician assistant staff assigned to the clinic. Assist with training, supervising, and evaluating clinic staff and volunteers.
  8. Perform administrative tasks required by clinic licensure, hospital privileges, and funding agencies.
  9. Assist in the general oversight of clinic operations to ensure effective health care, efficient working conditions, and client focused services.
  10. Coordinate with the Training and Volunteer Manager to meet volunteer needs of the program. Engages volunteers and/or students/residents to meet program needs; may include duties such as interviewing, selecting, supervising, training, and managing performance. Provides direct and timely performance and behavior feedback as necessary for volunteers and/or students/residents.
  11. Attend meetings and perform other duties and projects as needed




MINIMUM POSITION REQUIREMENTS:

 

Education & Experience: Graduation from an accredited school of medicine, successful completion of an accredited family medicine residency program and current California licensed, Board certified or Board eligible in Family Medicine. Demonstrated commitment to high quality health care for historically marginalized populations.

 

Special Skills/Equipment: Bilingual preferred. Knowledge and skills necessary to provide care appropriate to the age of the patients in the clinic practice including the principles of growth and development over the life span. Ability to assess data/information pertaining to patients age-specific needs. Ability to operate all clinical equipment. Effective and flexible communication skills and ability to encourage patient understanding of prevention practices and early diagnostic services. A valid California drivers license, proof of insurance and personal transportation are required. Current certification in basic cardiopulmonary resuscitation is required. Experience with electronic health records and ability to use or learn to use eClinicalWorks. Current CPR certification required.

Requires a moderate to high level of mobility, dexterity, and flexibility in order to perform essential functions. The employee is occasionally required to walk, bend, reach with hands and arms, and to lift 25 lbs. Must be able to sit or stand for long periods of time. Close vision is required. Work is performed in a typical clinic environment with moderate equipment noise. Occasional travel to other clinics and locations required. Ability to bend and move arms and hands in sufficient range of motion to complete physical assessment and type chart notes. COVID VACCINATION REQUIRED

Compensation depends upon experience.
