Advanced Practice Provider (NP/PA)

Community Health Clinics – Oregon

Salary: $61.00 – $78.90/hour

Status: Non-Exempt

Schedule: Full-time (primarily 8:00 AM – 5:00 PM, with one potential late shift per week depending on clinic)

About the Opportunity

This organization is seeking experienced and compassionate Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to join its growing team.

What You’ll Do

As an Advanced Practice Provider, you will deliver comprehensive, patient-centered care across a wide range of clinical services:
  • Perform comprehensive physical and psychosocial assessments and develop treatment plans
  • Diagnose, treat, and manage acute and chronic medical and behavioral health conditions
  • Prescribe medications and manage ongoing care for new and established patients
  • Perform in-office procedures such as suturing, biopsies, wound care, and fracture management
  • Order and interpret diagnostic tests including labs, X-rays, EKGs, and other studies
  • Provide patient education and coordinate care with community and social service partners
  • Participate in community outreach, committees, and interdisciplinary initiatives
  • Ensure compliance with federal, state, and local regulations and clinical best practices

 

Practice Environment
  • EMR: Epic (OCHIN Epic)
  • Patient Volume: ~9–10 patients per 4-hour session
  • Interpreter Services: Strong support (in-person, phone, and tablet-based)
  • Telehealth: Limited; primarily onsite care
  • Team Structure: Collaborative, multidisciplinary care teams
  • Work Environment: Structured, unionized setting

Schedule & Work-Life Balance
  • Standard clinic hours are Monday–Friday, 8 AM – 5 PM
  • One late shift per week (10 AM – 7 PM) at select clinics
  • Some clinics operate extended hours (up to 6 PM)
  • Limited telehealth flexibility in certain situations

Required Qualifications:
  • Board Certification as a Nurse Practitioner or Physician Assistant
  • Active Oregon licensure (or ability to obtain)
  • CPR/BLS certification
  • 1–4 years of clinical experience as an APP
  • Experience in community health or serving underserved populations. (preferred but not required)
  • Spanish language skills (preferred but not required)

Note: New graduate APPs are not eligible for these roles.
