ABOUT THE OPPORTUNITY
UHC Solutions is partnering with a well-established, well-funded community health organization with multiple locations across the South Bay region of Northern California to identify a Nurse Practitioner or Physician Assistant.

POSITION SUMMARY
The Clinician (NP/PA) delivers outpatient primary care services in collaboration with a supervising physician. This role is responsible for evaluating, diagnosing, and managing a wide range of patient conditions while promoting preventive care and long-term health outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conduct patient evaluations, including medical histories and physical examinations
- Diagnose and manage acute and chronic conditions
- Develop and implement individualized treatment plans
- Order and interpret diagnostic tests such as labs and imaging
- Prescribe medications and therapeutic interventions as appropriate
- Provide preventive care, screenings, and health education
- Coordinate care with specialists and internal support services
- Address behavioral, social, and environmental factors impacting patient health
- Maintain accurate, timely, and compliant medical documentation
- Participate in care team collaboration, including daily huddles
- Contribute to quality improvement and patient care initiatives
- Provide guidance and support to clinical staff as needed
- Participate in on-call or after-hours support as required
QUALIFICATIONS
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Active, unrestricted California NP or PA license
- Current BLS/CPR certification
- DEA registration (or ability to obtain)
- Experience in primary care or community health preferred
- Strong clinical judgment and diagnostic skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a collaborative, team-based environment
