Overview
The Mental Health Advanced Practice Registered Nurse (APRN) provides comprehensive psychiatric and addiction-focused care in an outpatient, community-based setting. This role includes psychiatric evaluation, medication management, and treatment planning for individuals with mental health and substance use disorders. The APRN works within an integrated care model, collaborating closely with primary care providers, behavioral health clinicians, and care coordination teams to support stabilization, recovery, and long-term wellness.
Responsibilities
- Conduct comprehensive psychiatric evaluations and ongoing clinical assessments
- Diagnose mental health conditions and develop individualized, evidence-based treatment plans
- Prescribe, monitor, and manage psychotropic and addiction-related medications in accordance with regulatory requirements
- Provide medication-assisted treatment and support recovery-oriented and harm-reduction models of care
- Collaborate with primary care providers, therapists, care coordinators, and case managers within an integrated team
- Monitor treatment effectiveness, medication adherence, and potential side effects, adjusting care as needed
- Educate patients on diagnoses, medications, treatment options, and recovery goals
- Respond to urgent clinical needs and provide crisis intervention when appropriate
- Maintain accurate, timely, and compliant documentation in the electronic health record
- Coordinate referrals to internal services and community-based behavioral health or substance use programs
